Mental Health Services
Targeted
Service Coordination (TSC)
Adults with severe and persistent mental illness often
have trouble accessing basic resources such as food,
housing, medical care, and financial support. Our TSC
workers help participants navigate the sometimes
overwhelming process of locating, applying for, and
coordinating these fundamental resources. TSC workers help
participants achieve this by linking them with a variety
of community resources such as mutual support groups,
volunteer opportunities, vocational rehabilitation, Social
Security Plans for Achieving self-sufficiency, and many
more opportunities aiding recovery and integration.
Psycho-Social Rehabilitation
Participants can reduce the impacts of mental illness by
developing a variety of skills which help in leading a
full and healthy life. Many of these skills include
problem-solving, coping, communicating, and budgeting. Our
PSR staff helps participants determine the skills they
would like to acquire; then, they work one-on-one with
them--in both their homes and communities--to help them
master these skills.
Medication Management Services--Clinic
Medications are often helpful in managing some of the
difficult symptoms associated with mental illness. Our
psychiatrist will complete a comprehensive diagnostic
evaluation, assessing participant’s symptoms, preferences,
as well as psychiatric and medical history prior to
developing a medication management plan. Our psychiatrist
then meets with participants on an on-going basis,
assessing and evaluating medication effectiveness and
monitoring for potential side effects. In order to provide
for integrated care, our medication management services
are provided only to individuals enrolled in at least one
of our other services.
Individual Psychotherapy--Clinic
Our
therapists come from both Social Work and Counseling
perspectives. Each approach utilizes a variety of evidence
based treatment modalities to help clients work through
difficult situations in their lives. Our therapists
develop an individualized treatment plan based on the
individual needs and goals of each participant.
Participants with either Basic or Enhanced Medicaid plans
can qualify for this service.
Social
Security Representative Payee Services
Some participants find it so difficult to manage their
finances that they risk losing the ability to provide
themselves with adequate food and shelter. As a Social
Security Administration designated Representative Payee,
Mountain States Group (MSG) can provide financial
management assistance, ensuring that these basic needs are
met. Additionally, MSG provides financial skills training
to participants through their PSR program. This helps
participants improve their ability to resume management of
their finances and save for important purchases. This
service is available to those participating in our PSR
program, and is provided at no additional cost.
Crisis
Assistance Services
For
those participating in our Targeted Service Coordination
and/or Psychosocial Rehabilitation Programs, we provide
after-hours crisis support during emergency circumstances.
These can include situations where an individual is at
risk of hospitalization, loss of income or employment,
loss of housing, incarceration, or any other serious
emergency. This service is provided 24 hours per day, 7
days per week. For crises that occur between
Monday-Friday, 9am-5pm, participants should call their TSC
or PSR worker first. After hours, an on-call worker will
provide support by assessing the nature of the need then
doing one of the following: provide links to appropriate
resources, such as law enforcement, child/adult
protection, and paramedics; provide prompting on effective
coping skills, relaxation exercises, as well as techniques
for managing stress and decreasing symptoms.
Service Eligibility
All Mountain States Group Mental Health Services are
currently provided only to Medicaid eligible
participants. Targeted Service Coordination, Psychosocial
Rehabilitation, Crisis Assistance Services, Representative
Payee and Medication Management Clinic services all
require Enhanced Medicaid. Individual Psychotherapy Clinic
services may be accessed by those who only have Basic
Medicaid.
