Home Recovery
Team
What does the Home
Recovery Team (HRT) do?
The HRT will
develop short-term treatment/recovery plans in partnership with
individuals in crisis. The plans will include services that can be
provided by the HRT, existing mental health providers and natural
support systems. The goal of the HRT is to ensure safety and
stress empowerment. The HRT will conduct home visits primarily
during evening and weekend hours (2 p.m. – 10 p.m.) with the
expectation of daily contact. Home visits may focus on working
together on safety plans, developing post-crisis recovery plans,
jointly solving immediate problems and developing and assisting
individuals in carrying out their daily living skills. The plan
will also define indicators for when services are no longer
needed. The average length of service will be 7-14 days depending
on the need of the individual.
Who can access the HRT?
HRT works with
adults (18 or older) experiencing a mental health crisis as
defined by the individual. This may include situations such as
increased life stressors, mental health symptoms, or other issues
that can be addressed safely in a home setting. The HRT primarily
serves the Boise/Meridian areas. Some exceptions may be made based
on individual client needs.
Who works on the HRT?
The HRT is
comprised of Licensed Master level Social Workers, Counselors,
Bachelor level mental health workers and trained Peer Specialists.
How do individuals access the HRT?
All referrals
for the HRT will be reviewed and approved by the Region IV, Mobile
Crisis Unit (MCU). MCU staff will determine the nature of the
crisis; whether or not HRT services will meet the needs of the
individual and ensure that the individual meets minimum
qualification for the program. Individuals must be willing and
able to engage in a Crisis Resolution Plan and follow the rules
and expectations defined by the HRT. Services are provided on a
voluntary basis.
Will the HRT be a
replacement for mental health services already in place?
The HRT is not
intended to be a replacement for any mental health services that
are already in place. The HRT will work closely with existing
mental health providers to coordinate supplemental crisis services
intended to help individuals avoid higher levels of care.
What is the first
step?
If you are an
individual in need of crisis services through the Home Recovery
Team and are already working with a case manager or mental health
professional please consult with them about your needs. Your
mental health professional can help you make contact with the
Mobile Crisis Unit and arrange for you to be interviewed. If you
do not have a primary mental health provider or cannot reach your
provider please call the Mobile Crisis Unit directly at 334-0800
during usual business hours or at 334-0808 after hours and on
weekends. The MCU will gather the necessary information to
determine if the HRT will be a safe option for you.
